At some point in history, a system was designed to prevent any subway operator from crashing his or her railcar with the one in front.

My mother told me she found out about this doing research for a high school project of hers (over 20 years ago), it had already been working for a while: Just in case “an operator went nuts all of the sudden”.
It makes me wonder. When was this implemented?
